Alexander County residents are encouraged to complete the 2025 Community Health Assessment (CHA) survey, which is conducted by the Alexander County Health Department every four years. This process is required by accreditation and guides public health interventions and improvement plans within the community by looking at key health needs and issues.
This survey is an opportunity for residents of Alexander County to have a voice in the programs that the Alexander County Health Department works on for the next several years. The health department needs the public’s help obtaining as many responses as possible to better represent the community.

Please visit any of the three Alexander County Library locations or contact the Alexander County Health Department for paper forms. Please submit completed paper copies at the Alexander County Health Department Administrative Building Monday through Friday from 8:00 a.m. to 12:00 p.m. or 1:00 to 5:00 p.m., or request to have them picked up by emailing kgansrow@alexandercountync.gov. The health department is located at 376 1st Ave SW in Taylorsville.
